For around two decades, I've diligently read thousands of papers related to the lipid or LDL hypothesis — the idea that blood cholesterol and LDL particles are major causes of atherosclerosis.

But is the lipid hypothesis true?

Despite being the dominant theory of atherosclerosis, the evidence in support of the lipid hypothesis is surprisingly weak. Strong proclamations that cholesterol and LDL are "harmful" do not hold up under scrutiny.

To illustrate this fact, I've decided to make the case against the lipid hypothesis in a new book titled "The Lipid Hypothesis of Atherosclerosis: Why LDL Particles and Cholesterol Do Not Cause Disease."

In the book, I highlight the mass of data undermining the lipid hypothesis, including:

  • The beneficial functions of cholesterol and lipoproteins.

  • The autopsy and imaging data showing little to no association between cholesterol/LDL and atherosclerosis.

  • The human pathological evidence contradicting the idea that LDL particles trigger the disease.

  • The lack of evidence implicating a direct harmful effect of cholesterol deposition in the arteries.

  • The potential roles of modified lipoproteins and oxidative stress.

  • The limitations of animal models and what they really suggest.
